I, the undersigned parent/guardian, hereby acknowledge and agree to the following terms regarding my child's participation in the Paseo Baptist Church Soccer Match against Euro Soccer Schools on May 1st, 2026:
- Financial Agreement: I acknowledge the required R100 contribution (covering breakfast, lunch, and drinks) and agree to hand this in by Wednesday or Thursday prior to the match.
- Assumption of Risk: I understand that participation in soccer involves inherent risks, including but not limited to physical injury, and I voluntarily assume all such risks on behalf of my child.
- Release of Liability: I hereby release, indemnify, and hold harmless The Paseo Baptist Church, its Pastor, Deacons, Trustees, Coach Arthur, Youth Leader Roger, and members from any and all liability, claims, or demands for personal injury, sickness, or property damage arising out of or in connection with participation in this sports ministry.
- Medical Consent: In the event of an emergency, I grant permission to the team leadership to secure necessary medical treatment, and I assume responsibility for all associated medical expenses.
- Code of Conduct: In alignment with the Church Covenant, I will instruct my child to walk circumspectly, exhibit exemplary deportment, and avoid excessive anger or unsportsmanlike conduct.
